Download Header Template
The Download Header Template form allows the user to download the Header Template Word document to a specified folder on the user's computer.

Fields
- Save Folder
- The folder into which the file will be saved
- File Name
- File name with a Word document (.doc) extension
Action Buttons
- Downloads the new Header Template
- Closes the form without downloading the Header Template to the user's computer
