Download Header Template

The Download Header Template form allows the user to download the Header Template Word document to a specified folder on the user's computer.

Save Folder
The folder into which the file will be saved
File Name
File name with a Word document (.doc) extension

Action Buttons

Download
Downloads the new Header Template
Cancel
Closes the form without downloading the Header Template to the user's computer
